LPA 2 - Application to register a lasting power of attorney

If you have made a lasting power of attorney (i.e. you are the donor) or have been named as an attorney in one, you can use this document to register the LPA with the Office of the Public Guardian. There will be a fee payable to register the document and you should check with the Public Guardian to find the current fee. If the donor has made both a 'Property and financial affairs' LPA and a 'Health and welfare' LPA, you will have to register these documents separately. This document is only for use in England & Wales.

Important Notice
Do not use this version if you created your LPA using either the new LPA1H or LPA1F versions (if at any time you created your LPA using our service, or created your LPA by another means before 1 July 2015, you can use this version). If using this version of the LPA 2 form, it must be properly completed and sent to the Office of the Public Guardian no later than 31 December 2015.
