LPA 1 - Notice of intention to register an LPA

Before registering a lasting power of attorney (LPA), whether property and financial affairs or health and welfare, use this document to notify any 'named person(s)' (also known as 'people to be told') in the LPA of your intention to register. All persons named in the LPA must be notified before an LPA can legally come into effect.This document will create a copy of the form for each named person listed (up to a maximum of 5). This document can only be used in England & Wales.

Important Notice
Do not use this version if you created your LPA using either the new LPA1H or LPA1F versions (if at any time you created your LPA using our service, or created your LPA by another means before 1 July 2015, you can use this version). If using this version of the LPA 1 form, it must be properly completed and sent to the 'people to be told' no later than 31 December 2015.
